Insurance
Pre-Authorization
Please be sure to
contact your health insurance provider prior to coming in for your first
appointment. Many insurance companies require pre-authorization for mental
health benefits, especially if you are scheduled to see someone who is out
of network. Failure to contact your insurance company may significantly
reduce your benefits.
Our building is
secured after 6:30 p.m. and prior to 7:30 a.m. In the event you have
an after-hours appointment there is a telephone located to the right
of the main entrance. There are instructions on the telephone.
The phone will automatically dial to the
security desk. When the security desk answers there is no need to
identify yourself, just use the password "CFCG".
We ask that you
pay at the time of your visit unless other arrangements are made with your
therapist. Payment can be made by cash, check, or credit
card. We accept Visa or Mastercard. Checks should be made
payable to the Child and Family Counseling Group or CFCG.
In the event
your account carries a balance, statements are sent monthly approximately
the 5th of each month.
If you need to
cancel an appointment, we ask that you contact the office at least 24 hours
in advance to be released from financial responsibility for the
visit. Please consult with your individual therapist about their
cancellation policy.
If you are a
current patient please inform us of any address, phone number, or
insurance changes so that we may better serve you.
If you have
waited longer than 5 minutes after your scheduled appointment time please
check in with our secretary. Occasionally circumstances arise that
cause this to happen and we ask for your patience.
Please call
your pharmacy to request refills which are NOT controlled medications.
If you have refills remaining on your prescription, your pharmacist will
fill your prescription without contacting the doctor. If you do not have
any refills remaining, please request that the pharmacist fax
a request to your doctor. This is already standard
procedure for most pharmacies. Our secure fax number is (703) 385-8353.
Controlled
medications include: Adderall, Concerta, Dexedrine, Focalin, Metadate, and
Ritalin. These medications require a written prescription from your
doctor every month. Please contact the front desk (703) 352-3822 ext. 1
and leave a message for Dr. Clayton, Dr. Paggi, or Dr. Robbins requesting
a refill for your controlled medication. Remember to leave your daytime
phone number so that we may contact you when the prescription is ready to
pick up.
Mail
away prescriptions (Merck Medco and Anthem RX) should be requested at the
time of your scheduled appointment.
Please
allow 48 hours for ALL prescription refill requests.
